Job Description:
MindPath Care Centers, a division of Community Psychiatry, is
actively seeking a compassionate and skilled Outpatient Therapist
to join our team. In this role, you will provide personalized
mental health services including assessments, therapy, and
medication management planning, aligning with our mission to
deliver high-quality, patient-centered care. You will collaborate
with a multidisciplinary team dedicated to creating holistic
treatment plans that support healing and growth. This is an ideal
position for individuals passionate about mental health advocacy
who aim to help clients achieve wellness and balance in a
supportive outpatient setting. Conduct initial patient evaluations
and establish therapy goals Provide regular outpatient counseling
sessions to patients Develop and maintain treatment plans with
multidisciplinary teams Document and preserve treatment records and
patient progress Engage in continuous education to enhance therapy
skills and knowledge Support patients and families to educate them
on coping strategies Coordinate care with psychiatrists for
comprehensive treatment plans Strong interpersonal and
communication skills Proficiency in various therapeutic techniques
and modalities Ability to work independently and as part of a team
Experience with electronic health records Commitment to patient
