The Patient Care Coordinator plays a vital role in daily operations, serving as the communication and scheduling hub for our clinical team, vendors, patients, and families. As a member of our team, you will ensure timely scheduling, precise documentation, and excellent service in a fast-paced environment. Accuracy, efficiency, and bilingual Spanish/English communication are essential. Key Responsibilities Scheduling & Care Coordination Schedule and update visits for home health aides. Coordinate procedures, services, and transportation with external vendors and facility partners. Track transitions such as hospitalizations, discharges, and revocations. Equipment & Supplies Order Durable Medical Equipment (DME) and coordinate delivery and pickup with patients and vendors. Order medical supplies through Medline and maintain supply tracking and par levels. Medical Records & Documentation Manage and maintain medical records, including digital charting, filing, and secure document handling. Perform data entry in the EMR with high accuracy. Complete and organize physician orders, consents, IDG/IDT paperwork, and other compliance documents. Handle faxing , receiving, and routing documents electronically. Communication & Customer Service Answer multiline phones professionally and route calls appropriately. Assist field staff with patient needs Provide Spanish/English interpretation and translation when needed. Administrative, Quality, & Compliance Support Assist with QA tasks, including chart audits, document tracking and compliance follow up. Uphold HIPAA and agency privacy/security standards at all times. Perform additional duties and special projects as needed. Requirements: Qualifications & Requirements Fluent in Spanish and English (required). Based in Los Angeles Typing: Fast, highly accurate typist (60 WPM). Exceptional attention to detail and data accuracy. Professional communication and customer service skills. Strong phone etiquette and ability to handle sensitive situations with empathy. Proficiency with Microsoft 365 (Outlook, Word, Excel), efax systems, and multiline phones. Experience with EMR Consolo preferred (or willingness to learn quickly). Prior hospice, home health, or healthcare administrative experience preferred. Familiarity with HIPAA and hospice documentation processes. Physical/Work Requirements Ability to work full shifts at a computer and manage multiple digital tools at once.
